Forms

The Rostered Leader Profile (RLP) form is for use by everyone on the ordained and lay rosters of the ELCA, as well as those who are candidates for these rosters.

The Ministry Site Profile (MSP) is for congregations, agencies, institutions, and organizations related to the ELCA to describe their ministry and their search for a rostered leader.

See the Call Process Forms page on the ELCA’s Call Process site for more about submitting these forms online. Your synod call process administrator is the one to contact first for help; but if you still have technical problems, you can call the Churchwide Office phone number shown on the Call Process Forms page.

Exit Interview for pastors and Exit Interview for councils are helpful lists of questions. The dean of the conference (or synod staff person) meets individually with the pastor and the congregation council to review and assess the ministry.

Transition Administration Information For departing pastors, deans and council leaders, this outline will make it easier to pull together information on parish leaders and programs, records and reports, and pastoral care needs.

Letter of Certification of Parish Records The dean meets with the departing pastor to assure parish records are in good order. (NOTE: ELCA guidelines expect the Parish Register to be kept in book form as well as electronically.)

Covenant of Agreement for Departing Pastors This is to be completed by the departing pastor and congregation council, and filed with the bishop’s office.

Letter to Congregation Concerning Covenant for Departing Pastor This is to be published in the parish newsletter, worship bulletin, or other significant outlet for communication in the congregation.

Letter of Agreement for Term Call This is to be completed by the term-call pastor and congregation council, and filed with the bishop’s office.

Letter of Agreement for Intentional Interim Ministry This is to be completed by the intentional interim pastor and congregation council, and filed with the bishop’s office.
